SEBORRHEIC KERATOSIS

SHINGLES

VARICOSE VEINS

CANCER OR UNDERGOING TREATMENT

USED ACCUTANE WITHIN A YEAR OF APPOINTMENT

EPILEPSY

ON BLOOD THINNERS (doctor’s note will be requested for prescriptions.)

Allergies to LIDOCAINE OR EPINEPHERINE.
